The Florida Department of Health in Liberty County utilizes community data and information to deliver services and programs that improve the health of our community. Promoting good health and helping people learn about the health risks of chronic disease is one of our goals.
Through programs that focus on chronic disease and injury prevention, we educate and inform the public on a variety of health topics including healthy lifestyles, diabetes prevention, heart disease and stroke. We teach classes and hold seminars, attend health events, and design and distribute materials to improve health outcomes in the community.
Florida CHARTS
Florida’s Community Health Assessment Resource Tool (CHARTS) is a one-stop website for public health statistics and community health data in Florida. It provides access to a wide range of information, including health statistics like births, deaths, disease rates, population data, and behavioral risk factors
