Liberty County School Health Program is responsible for health appraisal, records review, and nurse assessment.  School Health staff are also responsible for providing the following screenings: vision, hearing, scoliosis, growth, and development, and provide health counseling, referral and follow up of suspected or confirmed health problems that may be discovered through the screening process and/or evaluation for a specific problem. School health staff meet emergency needs in each school; provide medication administration and medical procedures, along with providing a health care curriculum that includes education on prevention of communicable diseases.  School Health staff provide consultation with students' parent/guardian regarding need for health attention by an appropriate provider, and maintenance of appropriate records.

The School Health Program provides health services to all students in grades pre-k through twelve in Liberty County.  School Health Services are provided through a cooperative arrangement with the Liberty County School Board.

Services

Services provided include, but may not be limited to:
•    Vision Screening*
•    Hearing Screening*
•    Scoliosis Screening*
•    Height and Weight Assessments/Body Mass Index (BMI)*
•    Nursing Assessment
•    Immunization and Health Record Review
•    Health Counseling
•    Individual Emergency Health Care Planning for students as needed
•    Assist parents with health referrals and finding community resources

*Mandated screenings start in October and are conducted throughout the school year.
 
Mandated Screenings will be performed on the following grades:

Kindergarten-Vision and Hearing
First grade- Height & Weight w/BMI, Vision and Hearing
Third grade- Height & Weight w/BMI and Vision
Sixth grade- Height & Weight w/BMI, Vision, Hearing and Scoliosis

IMMUNIZATION REQUIREMENTS

Prior to entry, attendance or transfer to schools (K-12), each child shall have on file a
Florida Certification of Immunization, DH 680 Form documenting the following:

 Public/Non-Public Schools K-12:

 Four or five doses of diphtheria, tetanus, and pertussis vaccine
 Three, four, or five doses of polio vaccine
 Two doses of measles, mumps, and rubella vaccine
 Two or three doses of hepatitis B vaccine
 One dose of varicella vaccine (kindergarten effective school year
 2001/2002, then each year an additional grade)
 Two doses of varicella vaccine (kindergarten effective school year
 2008/2009, then each year an additional grade)

Public/Non-Public Schools Seventh Grade:

In addition to all other compulsory school immunizations, children
entering, attending, or transferring to the seventh grade in Florida schools
are required to complete the following:
One dose of tetanus-diphtheria-pertussis vaccine (Tdap) effective school
year 2009/2010, then each year the next highest grade is included.
